1. Do I have to make a copy of the identity documents of the persons with intellectual disability?

To be eligible under the PIDVSS, the person must be a Hong Kong resident holding the Registration Card for People with Disabilities (with indication of intellectual disability*) issued by the Labour and Welfare Bureau (Central Registry for Rehabilitation) or a medical certificate (certify the person is a person with intellectual disability or eligible for PIDVSS 2015/16). The medical certificate can be issued by the enrolled doctor. The sample of the identity documents are shown in the appendix of the Doctors' Guide.
 
You have to check the identity documents of the person of intellectual disability, and keep a copy of the eligible person's Registration Card for People with Disabilities or medical certificate for inspection by staff of the Department of Health.
 
*The term "mentally handicap" is also accepted
 
 

2. Can the parent / guardian / representative show only the photocopy of the intellectual disabled person's identity document?

No. Enrolled doctors have to check the original identity document to verify the intellectual disabled person's eligibility.

 

3. What kinds of documents are accepted as a documentary proof for Hong Kong resident status?

For persons aged 11 years or above, identity documents that can be accepted as proof of Hong Kong resident status include:
  • Hong Kong Identity Card; or
  • Certificate of Exemption
For children aged below 11 years, identity documents that can be accepted as proof of Hong Kong resident status include:
  • Hong Kong Identity Card;
  • Hong Kong Birth Certificate indicating that the holder's Hong Kong resident status "Established";
  • Hong Kong Re-entry Permit; 
  • Document of Identity for Visa Purposes bearing endorsement showing the holder is permitted to remain in Hong Kong; 
  • "Permit to Remain in HKSAR" (ID 235B) showing unconditioned stay in Hong Kong Special Administrative Region or that the holder is permitted to remain until a specific date; 
  • Valid travel document with endorsement showing "the right to land" in Hong Kong; "the holder was permitted to land" in Hong Kong; "previous conditions of stay are hereby cancelled"; holder's eligibility for Hong Kong permanent ID card verified; "Certificate of Entitlement to the right of abode in Hong Kong Special Administrative Region"; unconditional stay in Hong Kong Special Administrative Region had been granted; "permitted to remain until" or "permission to remain extended until" a specific date; and
  • Certificate issued by the Births and Deaths Registry for adopted children indicating that the holder's permanent resident status "Established".

4. Who should sign the the Consent to Use Vaccination Subsidy?

The parents / guardians of the persons with intellectual disability can sign the Consent to Use Vaccination Subsidy under PIDVSS. Regarding persons have no legal guardian, issues concerned are usually handled by their relatives to safeguard the benefit and welfare of the person.

 

5. What is the purpose of requiring parents / guardians to sign the "Consent to Use Vaccination Subsidy"?

Parents / guardians sign the consent form to indicate that he / she agrees to use the Government subsidy for vaccination and to have their child / ward's personal data transferred to the Government, Hospital Authority or other relevant healthcare personnel. Staff of the Department of Health may contact the parents / guardians to enquire about the information related to PIDVSS. Doctors have to keep the consent forms and related vaccination records for at least 7 years for future inspection, to ensure that public funds are properly used.
