- Where can eligible persons receive the influenza
vaccination? Do we need to make an appointment before the vaccination?
Only enrolled doctors can provide subsidised influenza vaccination. Information about doctors who have joined IVSS is posted on the IVSS website (www.ivss.gov.hk). Enrolled doctors also display the IVSS logo and mini-poster in their clinics. You may contact the clinic to check if it is necessary to make an appointment.
- Do parents have to take their child to get
the vaccination in person?
Not necessarily. A representative may bring the child to an enrolled doctor
for vaccination, provided that the parent/guardian has completed and signed
the Authorisation Form, including entering the representative's name and identity
document number in the form. The representative has to produce his/her identity
document for checking.
- What documents do I have to present when
my child gets the vaccination?
Parent/Guardian should present the following documents to the enrolled doctor:
- Hong Kong ID or Birth Certificate or valid travel document of the child
showing the child's Hong Kong resident status
- If the parent/guardian takes the child to the doctor in person, the
parent/guardian's Hong Kong ID card or valid travel document is required
- If an authorised representative takes the child to the doctor, the representative's
Hong Kong ID card or valid travel document is required, together with
a completed Authorisation Form signed by the parent
- The parent may bring along a completed and signed Authorisation Form
to the doctor's clinic, or may choose to fill in the form at the clinic.
The Authorisation Form is available at enrolled doctors' clinics, or downloadable
from the IVSS website (www.ivss.gov.hk)
- If the child is aged 6 years or above, bring along the child's Student Handbook / Student Card / Pick up Card and submit a photocopy to the doctor; or submit a stamped Pre-primary Institution Certification Form. The Pre-primary Institution Certification Form is downloadable from the IVSS website (www.ivss.gov.hk)
