  1. Can parents/guardians submit photocopy or fax copy of a completed Authorisation Form?

    No. The Government will only accept duly completed original copy of Authorisation Form for reimbursement claim. Parents/Guardians or doctors may photocopy blank forms for future use.

  2. Can a child still receive subsidised influenza vaccination if parents/guardians have left out some information on the Authorisation Form?

    Provided that the parent/guardian fully understands the content and has signed the Authorisation Form, clinic staff may help to complete the form.

  3. Can parents/guardians or the representatives show only the photocopy of the child's identity document?

    No. Doctors have to check the true copy of identity document for verifying a child's eligibility.

  4. Can parents/guardians provide a f ax copy or photocopy of the Pre-school Institution Certification Form?

    No. Parents/Guardians have to submit original copy of the Pre-school Institution Certification Form.

  5. Are children not born in Hong Kong eligible for subsidy? What kinds of documents are accepted as a documentary proof for Hong Kong resident status?

    Children are eligible for subsidy if they are Hong Kong residents regardless of the place where they were born. Identity documents that can be accepted as proof of Hong Kong resident status include:
    • Hong Kong Permanent Identity Card;
    • Hong Kong Birth Certificate indicating that the holder's Hong Kong permanent resident status "Established";
    • Hong Kong Special Administrative Region Passport;
    • Hong Kong Re-entry Permit;
    • Document of Identity for Visa Purposes bearing endorsement showing the holder is permitted to remain in Hong Kong;
    • "Permit to Remain in HKSAR" (ID 235B) showing unconditioned stay in Hong Kong Special Administrative Region or that the holder is permitted to remain until a specific date;
    • Valid travel document with endorsement showing "the right to land" in Hong Kong; "the holder was permitted to land" in Hong Kong; "previous conditions of stay are hereby cancelled"; holder's eligibility for Hong Kong permanent ID card verified; "Certificate of Entitlement to the right of abode in Hong Kong Special Administrative Region"; unconditional stay in Hong Kong Special Administrative Region had been granted; "permitted to remain until" or "permission to remain extended until" a specific date;
    • Certificate issued by the Births and Deaths Registry for adopted children indicating that the holder's permanent resident status "Established".

    Accepted documents for proofing Hong Kong resident status are shown in the appendix of the Doctors' Guide at the IVSS website (www.chp.gov.hk/files/pdf/Doctors_Guide_3rd_Appendix.pdf).

  6. Are all children required to provide pre-school attendance proof?

    This is applicable only to children aged 6 years or above but attending a kindergarten or child care centre. Parents have to show the attending doctor the child's pre-school attendance proof such as Student Handbook or Student Card and submit a photocopy, or submit a stamped Pre-primary Institution Certification Form.

  7. Can grandparents or relatives sign the Authorisation Form?

    If grandparents or relatives are the child's legal guardians, they can sign the Authorisation Form. If not, parents must authorise the grandparents or relatives by filling in the Authorisation Form beforehand.

  8. Do I have to keep a photocopy of the Authorisation Form in the clinic?

    IVSS does not require enrolled doctors to keep the photocopies of Authorisation Forms received, but for future checking purpose, please retain the relevant vaccination medical records or log books.

  9. Do I have to make copy of the identity documents of children and their parents/ guardians/representatives?

    Enrolled doctors have to check the identity documents of the children and their parents/guardians/representative, but it is NOT necessary to keep copy of their identity documents.

  10. Can I correct the mistakes on the Authorisation Forms made by parents?

    You can help the parents to correct the mistakes on the Authorisation Forms after checking the information. Then you or the parents sign against the corrections.
