

(1) Background
The Department of Health (DH) is conducting a territory-wide population health survey (PHS) to understand the health status of the Hong Kong population, their health-related lifestyle behaviour and practices, and to collect other important health information. The survey commences on 18 September 2025 and is expected to last for 15 months.
(2) Objectives
To strengthen the Government's information base to assess health status of the population of Hong Kong.
(3) Target population
Hong Kong residents aged 15 or above
(excluding inmates of institutions, persons living on board vessels, foreign domestic helpers and visitors)
(4) Contents of PHS
The PHS comprises two parts:
A. Household Survey ![]()
DH has commissioned the MOV Data Collection Center Limited to conduct the PHS 2025/26. A sample of quarters is selected by DH using a scientific method. Invitation letters will be sent to the selected quarters by post. Household representatives and household member(s) shall read “Personal Data Collection Statement” and “Terms & Conditions”.
Online questionnaire ![]()
Household representative can activate the online questionnaire system (via https://phs.portal.ehealth.gov.hk) using the household reference number and activation key printed on the invitation letter to complete online household questionnaire. The household reference number and activation key are unique to each selected quarter and is not transferrable.
After completion by the household representative, all eligible household members aged 15 or above can continue to complete the online individual questionnaire by logging into the online questionnaire system themselves. The individual questionnaire will ask about each respondent's physical and psychosocial health status, health behaviour and preventive health practices.
Household interview ![]()
If necessary, household representative and/or household member(s) can make an appointment by phone or via online platform to request for face-to-face interview. The interviewer will assist to complete both household questionnaire and individual questionnaire.

If the online questionnaire is not completed by the deadline as stated in the invitation letter, an interviewer from the MOV Data Collection Center Limited commissioned by the DH will visit the household to assist all eligible members aged 15 or above in the household to complete the online questionnaire. The interviewer will wear a vest uniform in teal colour with the “Population Health Survey” and the “Department of Health” logos. The interviewer will also carry an identity card issued by DH for identification purpose.
B. Health Examination ![]()
DH has commissioned the KingMed Diagnostics (Hong Kong) Limited to provide health examination service for PHS 2025/26. All respondents aged between 15 and 84 who completed the questionnaire will be invited to sign a consent form for health examination. Randomly selected respondents will be further invited to undergo health examination at the designated centers of the KingMed Diagnostics (Hong Kong) Limited located in Tsuen Wan, Mong Kok, Tsim Sha Tsui, Causeway Bay and Central. The health examination will take about 45-60 minutes and is free of charge. Free health examination reports and health advice will be provided as appropriate.

To follow-up on the health examination results, participants can choose to visit their family doctor or District Health Centre/Express (DHC/E) for subsequent care. For more information on DHC/Es, please visit www.dhc.gov.hk.
Note: If you have been selected to participate in the health examination, you will receive a phone call from the KingMed Diagnostics (Hong Kong) Limited within six months of completion of the household interview to schedule an appointment, with caller number 2919 7236.
(5) Handling of data collected by PHS 2025/26 ![]()
All data collected will only be used for research and statistical purposes. Survey results will only be released in aggregated form. All personal data and contact information collected will be kept strictly confidential and disposed of safely within 12 months after completion of the survey. Personal identifiable information will NOT be disclosed to any unauthorised party without your consent. For details, respondents of PHS 2025/26 can read the “Terms and Conditions” and “Personal Data Collection Statement” in the online questionnaire system.

If your household is invited to take part in PHS 2025/26, please give us your generous support. Your active participation is crucial for improving public health in Hong Kong. As a token of appreciation, DH will provide a cash coupon to each eligible household member who completed the questionnaires. Additional cash coupons will be provided to those who are eligible and have completed the health examination.

(7) eHealth registration and sharing of data
Electronic Health System (eHealth) is a territory-wide, patient-oriented electronic sharing platform which enables authorised healthcare providing organisations in the public and private sectors to access and share participating patients' electronic health records for healthcare purposes. If respondents have not registered for eHealth, our interviewer can assist them to register during the household visit, to build a lifelong and comprehensive personal electronic health record, enable healthcare professionals to provide them with continuous healthcare services, and enable them to manage their own and their family members' health records through the eHealth App.
After completing the questionnaire, respondents can choose to deposit the health information collected through PHS 2025/26 into their “Personal Folder” in eHealth, enabling them to retain and utilise relevant health information. The DH will provide advice and referral letters to those with abnormal test results in the health examination. The test reports will also be deposited in their personal eHealth accounts, enabling them to seek appropriate follow ups and treatment from their family doctors or district health centres.
